- Click HERE to access the renewal form for adult single/family memberships.
- Fill out the renewal form completely - YOU DO NOT need to sign it if sending in an email.
- Save the completed renewal document to your computer
- Email the completed renewal form (as an attached document) to the club email: membershipHSA@gmail.com
- Click on the corresponding membership option below, and then "buy now"
- YOU MUST write your name (or the person that you are paying for) in the notes section at the end of your transaction after putting in your payment information.
Your renewal is complete ONLY when:
- You have paid online (and written the names of the individuals in the PayPal notes)
AND
- Submitted your renewal form either in-person or via email within 14 days of sending payment
*** Your new membership cards will be mailed to you once the form has been processed by the membership secretary.

One year renewal for a single membership - with an additional $3.50 to help us cover processing fees.
